It was determined through the course of the investigation that a caregiver committed acts of prohibited punishment on a 2 year child in care when they pulled the child's hair, poked them in the face and kicked the child.

It was determined through the course of the investigation that a 2 year child was made to sit in time out for longer than 2 minutes.

Upon review of caregiver files, 2 caregivers, taht were hired more than 90 days ago, were lacking pre-service training in required topics.

A caregiver in the toddler classroom was present at the operation before receiving notice from the CBCU regarding the their eligibility.

It was determined through the course of the investigation that a caregiver with a conditional status was left alone to supervise a group of children.

A 22 month old child in care was administered .62 ML more of a dosage of ibuprofen than what was directed on the medication's label for the child's age, per the parents request. This was corrected at inspection when the operation made a change in policy and sent a mass email to all enrolled parents stating that the operation will no longer administer medication effective 4/24/2024.

A 10 month old was observed sleeping in a crib with a blanket over them. This was corrected at the inspection when the blanket was removed from the infant and the crib.

A child with a diagnosed food allergy to peanuts was lacking an allergy emergency plan on file.

Infant feeding forms were not being updated every 30 days.

An infant in the infant room was not buckled into the kidney bean toddler feeding table. This was corrected at inspection when the caregiver buckled the child in.

Medication that was provided by parents was not labeled with the date that it was brought to the operation. This was corrected at inspection when the caregiver labeled the medication with the date.

A child's file was lacking a health statement and immunization record.

Annual Sanitation inspection has not been conducted for the current year. The most current documentation at the center is from 03/22/2021.

Director was lacking required annual training hours in required topics. Multiple caregivers were lacking annual training hours in required topics.

At the time of inspection an infant was found to be in a swing, when teacher was asked how long the infant had been in the swing, the teacher said the infant had been in there for 40 minutes. Infant was removed from the swing at the time of inspection.

The operation did not have current documention for fire drills for the month of March 2022 and the month of April 2022.

A medication form was signed authorizing the caregivers to administer OTC pain reliever in excess of the container dosing instruction on the bottle. This was corrected during this inspection when the operation agreed to no longer administer the medication with out a doctor note.

It was determined the director has not obtained all annual training hours.
