Four out five employee records reviewed for their annual year were missing the required trainings hazardous materials, building safety, allergic reactions, medication administration emergency preparedness, and communicable diseases.

Last fire drill and Co Detector checks were documented on 02/2023. Per provider checks and drill were conducted March and for April last week but not documented. Note: This was corrected at inspection when provider documented the drill and checks during inspection.

Trash cans in the restrooms were observed without lids.

Merry-go-cycle is within 2-4 ft feet from two metal poles.

Diaper changing mat in the older infant room is torn.

Sideboard in the restroom within school-age room #1 is lose exposing some of the wall material.

There are protruding bolts underneath the play structure in the playground. There are two utility cables exposed in the playground. There is gap in the fence between 4-5 inches wide. Note: During inspection provider cut the bolts and fixed the fence.

Two out of seven staff training records reviewed is missing transportation training.

A written list of the children that attended a field trip and a copy of the allergy plan were not taken on the field trip.

Chipping paint was observed in different classroom including prek and school-age classrooms. One of the prek classroom has missing side boards exposing parts of the wall materials. There is gap on the floor by the door frame diving the two toddler classrooms.

30 children ages 2-8yo were supervised by 5 staff members during a field trip to the pool.

A 12 passenger van used for transportation is missing an electronic child safety alarm.

Use zones surrounding a spring see-saw and merry-go-cycle did not meet the required six feet. Note: This was corrected during inspection when provider removed a merry-go-cycle and see-saw from playground.

A 7 passenger van used for transportation is missing a fire extinguisher.

No CO detector nor fire extinguisher checks were documented for 2022. Note: This was corrected at inspection when provider conducted and documented checks.

A fan in the infant room was observed with dust build-up and there are two broken tiles in the infant room. Note: fan was removed from classroom and tiles were covered with a mat during inspection.

This deficiency is not related to investigation: Observed a caregiver supervising 5 infants ages ranging from 3 months to 11months. NOTE: Specified age group is 10 months old. Child/Caregiver ratios 4:1.

Playground slide structure is missing six inches of loose fill surfacing.

Based on the information gathered, facility did not maintained children's sign in and out for week of 10/4/21 to 10/8/21 for one classroom.
