Ten staff records were reviewed. On file is missing up to date training certificates for health / safety topics. A second file is missing high school diploma / equivalent. A thrid file (contractor) is missing employment information (hired date and orientation / employee and center policies statements), notarized affidavit (form 2985) and preemployment affidavit (form 2912).

A caregiver's complete training record was not available for review at the operation during the inspection.

A caregiver's Pediatric CPR training certificate and letter did not include that the training also included Pediatric First Aid Training. Note: This was corrected at inspection when the pediatric cpr and pediatric first aid certificate was obtained.

A hole was observed at the bottom of the fence in the after school outdoor playground. The hole was large enough for a small animal go through and have access to the playground. A utility room with cleaning supplies was unlocked in the after school playground. Note: This was CORRECTED during the inspection as the hole was filled with dirt and the utility room was locked.

The Director's time sheet was not available for review.

The sign in and out log did not have the dates for the week and employee/ parent's initials. Corrected at Inspection: Dates and Initials were written in the log.

Two play yards used by infants have a mattress that is no longer firm and flat. There are gaps between the mattress and the play yard sides.

The vent in the after-school room was observed with dust and debris.

A can of air freshener spray was observed on top of the toilet in the children's afterschool restroom. A second can of air freshener spray, and several plastic bags were observed under the sink in the afterschool room. The products and plastic bags were accessible to children. Several spoiled oranges, and children's masks were observed on the ground in the afterschool outdoor playground. Note: This was CORRECTED during the inspection as the air freshener sprays and plastic bags were removed and stored in an area out of children's reach. The children's facial masks and spoiled oranges were discarded.

A play yard was observed without a sheet in the infant room. Note: This was CORRECTED during the inspection as a fitted sheet was provided.

The director's certificate of training on transportation safety was not available at the center. As per operation the certificate was located at another center. Note: This was CORRECTED during the inspection as the certificate of training was obtained.
