One caregiver had the care of 17 children between the ages of 4 months and 10 years old. There were 5 infants in the group. Two more caregivers were present in the center and the groups were seperated during the inspection.

Observed diaper changing mat in the infant room torn at the time of inspection.

A review of the Employees Who Left Employment in 2023 data from January 22, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

Annual fire inspection was not current. Last conducted on 6/30/22.

Transportation training has not been updated. Last updated 2022.

Three out of Four files reviewed was missing: Name and address of emergency contact information, telephone number of the person to contact in case of emergency, and date of admission.

Staff files were evaluated and new staff member has not completed the last 16 hours of pre-service training. Staff was hired on 2/15/23.

Observed a bottle of medication in the afterschool on the table accessible to the children at the time of inspection. NOTE: CORRECTED AT INSPECTION as medication was removed and inaccessible at the time of inspection.

Observed one staff present in the afterschool classroom supervising 13 children between the ages of 2 years old to 6 years old while they were eating their lunch while they were watching the television. NOTE: CORRECTED AT INSPECTION as the television was turned off at the time of inspection.

Observed one staff present in the afterschool classroom supervising 6 children between the ages of 12 months to 6 years old. The youngest to the older children was more than 18 months difference.

The child files were incomplete. Two files were missing phone numbers for the people who are authorized to pick the children up. These were added during the inspection. One file was missing a health statement. One file was missing an immunization record.

Staff files were incomplete. Three files were missing high school diplomas. One file was missing an identification. A copy of the ID was added during the inspection. Three affidavits of employment were not notarized. One file was missing the employment begin date, receipt of center and personnel policies and orientation.

The new cots have been ordered but have not yet arrived.

An infant's milk bottle was propped on a blanket as she fed from it while sitting in her car seat.

The employee list was not current. Employees present at the inspection were no included in the list.

None of the children present were signed in to care.

The cots have noticeable wear. A cot in which a child was napping had only one side of the mesh attached to the frame. A second cot which was set up but not occupied had fraying on the mesh which attaches to the frame.
