One infant did not have a current feeding schedule at review. *Parent emailed the feeding schedule within an hour*

The lightning in the classroom was insufficient to see the children's faces. This was corrected when the hallway light was turned on, allowing light to enter the classroom.

Caregiver did not know many children they were responsible for during nap time.

There were forty-two children to one caregiver during nap time. Note this was corrected when the twelve children that were awake were transitioned to another room to be supervised by a caregiver.

A child had a serious self-inflicted injury in a sensitive area of the body and parent was not notified immediately.

There were two caregiver caring for 6 infants and 31 children ages 18 months and up. This was corrected when caregivers transitioned their student to their individual classrooms.

Infants were grouped with children of 18 month difference. This was corrected when the caregivers transitioned the children to their rooms.

Children were not fastened with the safety straps on the high chairs. This was corrected when the caregiver strapped the children with the safety straps.

The caregiver did not wash her hands after wiping the child's nose. This was corrected when the caregiver was guided to wash her hands.

Children were eating cheerios directly from the table. This was corrected when caregiver threw away the cheerios and provided new cheerios on top of a napkin.

The children were restricted in the highchair for 19 minutes. This was corrected when the caregivers removed the children from the highchairs.

Outlet in the 18-24 month classroom was uncovered. This was corrected when the director covered the outlets.

There were a total of 7 children to one caregiver however the specified age group is17 months therefore caregiver was over ratio by 2 children. Note: This was corrected when another caregiver joined the classroom.

A person with a pending eligibility status was observed in direct contact with children. This was corrected at inspection when the caregiver left the building.

Due to construction, the infant class temporarily moved to a room without a sink. This was corrected at inspection when the class relocated into a room with a sink.

The daily report for a 17-month-old shows the infant was changed once at 10:59 am on 7/9/2024 resulting in a severe rash. No other diaper changes were logged for the child that day.

A caregiver stepped out of the classroom to answer the door leaving the children unsupervised.

A classroom had 11 children between 10 months - 22 months with one caregiver during naptime. The specified age group was 12-17 months with the caregiver being over ratio by 5 children.

A ball play item on the play structure had several broken balls. Corrected at inspection: The balls were taken off.

The caregiver failed to ensure a used diaper wipe was discarded after use and accidentally left it in a child's clean diaper.

A caregiver without a completed background check was observed to be in a classroom. **This was corrected at the inspection when the caregiver left the premises.**

There was a broken plastic portal in the outdoor climbing equipment which had a sharp hole in it that presented a hazard for entrapment. This was corrected at inspection when the broken portal was removed from the equipment.

Two infant caregivers did not have proof of training available to review in the required topics of Shaken Baby Syndrome, SIDS, and Safe Sleep.

One child's record was missing the required Emergency Care Authorization.

There was one television in the infant room that was not anchored or secured. There was one television in the after school room that was not anchored or secured. This was corrected at inspection when the director had the televisions removed from the classrooms.

There was no soap or paper towels available in the boy's and girl's toddler restrooms. This was corrected at inspection when the soap and paper towels were stocked.

At the time of inspection several staff in direct care of a group of children were using their cell phones.

Three employees were working in the center without a current background check and current FBI fingerprints.

Two caregivers in the infant room were not certified in SIDS and Shaken Baby Training.

Based on the observations made during the inspection, there were 6 children ages 12 to 18months old with one caregiver. There were 23 three and four year old children with one caregiver. This was corrected when a second caregiver arrived in the classroom. This was corrected when the classes were split and a second caregiver took the other class.

Based on the information gathered, the center director took all the children's records away from the center to keep them safe from another person in the center.

At the time of inspection the caregvier in the 12 to 17 month old classroom had 6 children. This was corrected when the director took the oldest child to the next room.

At the time of inspection one infant with a wedge under the mattress without a sleep exception form on file. The wedge was removed.

The pea gravel and the mulch around the fall zones of the play structure was not maintained at its proper depth.

Operation staff did not use good judgment when they placed children in the front bucket of a large tractor which was raised between 4-6 feet above the ground.
