Observed 4 staff files out of 6 staff employeed. 3 caregivers did not have the pre employment affidavit in their files.

Observed a glass sliding door without any markings to show that it is a glass door. This was corrected when the director Marked the door in the childrens eye level to show it was a door.

Reviewed the operational policies and the Criteria used for extreme weather was not found in the policies.

A caregiver did not know how old the youngest child in the room was. This was corrected when the director told the caregiver the age of the youngest child.

There was not an August fire inspection documented showing it was completed. This was corrected when a fire drill was completed during the inspection.

Observed bottles without being labeled with the infants first name and last initial. This was corrected when the caregiver labeled the bottles..

The oepration could not show proof of the playground checklist for the last week of August. I spoke with the director about completing the checklist daily before children go outside. The checklist was completed for today.

Two slides were seen in outdoor play area without any loose fill. This was corrected at inspection when staff removed the slides and placed them in area inaccessible to children. The slides were removed from the playground.

A classroom of children were left unsupervised when the caregiver answered the front door for licening. The caregiver returned to the classroom.

There were two slides on the toddler playground, The use zone measured from the stairs of the slide to the brick wall was 3.9 feet and from the slide to the fence was 4.5 feet. The small slide had a use zone of 3.2 from the slide to the fence. These items were removed from the playground.

One caregiver did not know how many children that they had in their care. Licensing counted the children while in the classroom and told the caregiver how many children there were.

Observed a bucket full of mop water that was in a closet that did not have the ability to be locked. This was corrected when the mop bucket was removed from the closet.

It was found that a classroom was left unsupervised during nap time.

Observed 2 caregiver files, the following was missing from the files; One caregivers file was still missing proof of education and there were only 12.5 hours of training completed from November 2022-November 2023. A second file was still missing the Proof of education and Notarized affidavit.

Observed three caregivers counted in ratio and there was not another caregiver in the building. I spoke to the owner and director about this standard. Another person showed up at the operation during my inspection.

A classroom of children were left unsupervised when the caregiver answered the front door for licening and Door Dash employee. The caregiver returned to thee classroom.

A caregiver did their CPR online and did not complete a hands on portion of the CPR. I spoke to the caregiver about the guidelines.

Observed two caregivers counted in ratio and there was not another caregiver in the building. I spoke to the owner and director about this standard.

A child was sitting in a highchair and was not fastened with the safety strap.The child was fastened into the seat during the inspection.

There were two outlets in the infant room that was uncovered.

Observed 2 caregiver files, the following was missing from the files; One caregiver file only had 6 1/2 hours of training and no other documents for that caregiver. A second file was missing a photo ID, Proof of education, Notarized affidavit.

There was an 13 month old infant in the same room as 4 year old children. There was a 35 month gap between the youngest infant and the oldest child in the classroom. This was corrected when the infant was taken into another room.

A caregiver had stepped away from classroom leaving children without supervision during inspection. This was corrected at inspection when caregiver came back into the classroom.

The fire inspection was last completed on 04/27/2022.

There were 2 infants sleeping with bottles in their cribs. This was corrected at inspection when caregiver removed bottles from cribs.

The fire extinguisher observed at facility has not been serviced since April 2022.

There were two slides on the playground that did not have any loose fill around or under the slides.
