While reviewing video footage it was observed that a caregiver in a toddler room did not wash their hands after conducting a diaper change for two children.

While reviewing video footage it was observed that a caregiver in a toddler room did not wash childrens hands after conducting a diaper change for two children.

Based on the information obtained during the investigation, it was determined a caregiver used inappropriate treatment of a child when the caregiver roughly picked up a child by the arms and seated the child on a chair.

There were four immunization records missing out of the 10 children's files reviewed.

Six out of the ten staff files were missing their Affidavits.

Caregiver did not ensure to check if child received an injury as she did not observe the affected area after child was hit with a book in the head.

Annual Fire inspection was not cunducted. Fire inspection dated 8/10/23 with expiration date of 8/10/24.

One record did not have immunization records in files.

Transportation Vehicle evaluated with no Child Safety Alarm installed. Vehicle purchased in 2019.

Most recent Sanitation inspection was not available for review.

Employee training hours evaluated to be missing trainings for, Preventing and controlling the spread of communicable diseases, including immunizations. Administering medication, if applicable, including compliance with ?746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?). Preventing and responding to emergencies due to food or an allergic reaction; Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Classroom with children from 23 months - 2 years old were watching a video. Note: This was corrected during inspection as screen was turned off and manipulative's were given to the children.
