During inspection backyard gate was observed broken in several sections.

During the inspection it was observed that the most recent inspection from the last licensing inspection was not posted. This is a required posting.

During the inspection it was found that the assistant caregiver only had completed 7/24 of their required annual training hours.

During the inspection it was found that the primary caregiver had only completed 8/30 of their required annual training hours.

During the inspection it was observed that broken pieces of a fence and of a trampoline were on the part of the playground that the children have access to.

The center did not have an updated CPC posting. The last posting was dated 9/2021.

4 children's files reviewed did not have a health statement filled out. 4 children's files reviewed did not have the Special Needs section filled out.

One infant in care did not have feeding instructions signed by a parent.

A fire drill had not been conducted since March 2022.

One infant still on baby food from home did not have an infant feeding instruction signed by a parent.

Verification was not available for review during the inspection to support permission is given to leave the daycare to ride with the provider to pick up children from their elementary school.

The daycare area is located in a converted garage with the 2nd exit through the kitchen area.

The fire extinguisher available for review during the application inspection was not mounted.

The operational policies available for review did not include procedures for parents to review a copy of the Minimum Standards or to review the most recent copy of the licensing inspection report.

The primary caregiver did not have verification of completing safety training prior to transporting children.
