During inspection, it was found that required annual training was not current. Four staff records were reviewed: All staff need the following: Emergency Preparedness Medication Building and Premises Safety Communicable diseases Food allergies Hazardous Materials CG and J need SIDS/Shaken Baby/Abusive Head Trauma CG needs 5 hours more child development and 2 business management Judy needs 12 hours of child development - the required training above can count towards these hours.

An infant was observed on the floor with a bottle and a toddler was observed walking around with a sippy cup.

Toys, shelves, tables, chairs, and equipment including the floors need to be wiped down thoroughly and sanitized by either soaking, wiping, sweeping/mopping with a sanitizing solution such as bleach solution or hospital grade cleaner.

During the investigation it was determined that infant daily sheets are not sent home each day.

During the inspection I observed the restroom that the children use and found that a cabinet within the children reach was not locked and had pine sol and bleach in children's reach.

During the inspection I observed drywall falling out and paint chipping in the after school bathroom, as well as this I observed old food particles and dead bugs on the floor of multiple rooms.
