One newly hired caregiver did not have proof of orientation.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

There were multiple children observed eating without water served for lunch.

A classroom was observed to have 43 children present with only 2 caregivers available.

The lighting inside one of the classrooms was not adequate as eyes had to adjust when walking into the room.

Ceiling tile above hand washing sinks in sunshine room has a Y shaped crack in the corner. The tile will begin to dip.

There were 3 toddler cups in the sunshine room that were not labeled with any child's name.

9 outlets in last gym area were not covered.

Play structure on gym floor with two red slides has two areas with large cracks. One large crack on first platform is approximately 12 inches long in a zig-zag shape with two peaks. Second large crack is deep and circular on second step to one slide.

2 out of 9 caregivers do not have valid CPR training that is required to be taken in person.

While toddler caregiver was taking children to classroom one child was out of site on top of a play house structure in the open gym area approximately 4 feet off the ground. The child could of fallen off the top and caused a serious injury.

Plug located on back wall of last gym at the entrance to the carpeted area sparked twice as a 6 year old child plugged a speaker into the outlet. Child repeatedly played with plug of speaker and outlet prior to plug sparking.

There is a large hole behind a toilet located in the toddler classroom. There is a pipe going into the wall from the back of the toilet. The hole is approximately 7 inches tall and 6 inches wide. Note: Photo was taken.

One caregiver walked out of the room to take a child to the restroom leaving the other caregiver with 26 children that had a specified age group of 3 years old.

There were four napping mats that had tears in them that do not allow them to be waterproof.

A class was left unsupervised when the caregiver walked into another class to let a child from a different group go to the restroom. Children were also unsupervised when they were allowed to go the restroom in a different room, while the caregiver maintained the rest of the class.

The carpet in the back classroom was pulled up near the entrance of the mat area caused a tripping hazard.

The front playground had less than six inches of mulch present and there were weeds that had grown in the playground.

Napping equipment in the naproom did not provide access to the walkway nor did it allow the caregiver to be able to walk inbetween each mat.

The operational policies did not include a policy on suspension/expulsion nor vaccine preventable disease policy for staff. The abuse and neglect policy was incomplete.

Documentation of monthly building and grounds checklist could not be provided.

There were three unwrapped water hoses near the playground that pose a tripping hazard.

Documentation of current transportation training for the directors could not be provided.

One caregiver does not have current First Aid nor CPR.
