One caregiver was missing four hours out of the required 24 hours of annual training.

A playground checklist had not being completed for June. This was corrected during the inspection when the director checked the playground and completed a playground checklist.

Out of four children's records reviewed, one child needed an updated immunization record.

The children's product recall verification form was last signed in May 2024. This was corrected during the inspection when a new form was completed.

One infant's feeding instructions had not been updated since May.

In the infant room, children under the age of two were watching a television program on the caregiver's tablet. This was corrected during the inspection when the caregiver was informed that children under two cannot have screen time and the caregiver turned the tablet off.

There was a bottle of Clorox wipes and a bottle of Febreze on the shelves where children could reach. This was corrected during the inspection when the supplies were moved to a closet that the children could not access.

One caregiver had only completed 12 out of the 24 required annual training hours and another caregiver had only completed 16 hours out of the 24 required annual training hours for the 2022-2023 training year. This was corrected during the inspection as the training period has ended and both caregivers have started obtaining their hours for the current year.

The director had only completed 9 out of the required 30 hours of annual training for the 2022-2023 training year. This was corrected during the inspection as the training period has ended and the director has already obtained over 30 training hours for the current year.

The first aid kit was missing adhesive tape.

An infant was observed sleeping with a bottle. This was corrected during the inspection when a caregiver removed the bottle from the crib.

One caregiver and the director had expired first aid and CPR training.

The center's operational policies did not address all of the topics required in minimum standards.

Out of six children's files reviewed, five were missing admission dates, three school-age children were missing school information, and one school-age child was missing permission to ride the bus from school.

During the inspection observed caregiver prepare a bottle on the diaper changing table and then change on diaper on the table.

During the inspection observed a caregiver change a diaper, turn her back and throw a diaper in trash can leaving infant buckled on diaper changing table.

During the inspection a 9 month old was observed in a bouncer drinking a bottle that was propped up by a blanket. This was corrected by the caregiver removing the bottle and taking the infant out of the bouncer.

Observed two infants under 12 months old sleeping in playpens with blankets covering them. This was corrected by the caregivers removing the blankets.

During the inspection the personnel files were missing, notarized affidavits, high school diplomas, and documentation of pre-service trainings.

Observed one infant sleeping in a bouncer with a blanket. This was corrected by the caregiver removing the infant and the blanket.

During the inspection there were eight infants observed sleeping, two infants did not have a crib/playpen and were observed sleeping in a bouncer. This was corrected by the director taking two playpens to the infant classroom.

3 out of the 5 staff employed longer than 90 days and were present did not have a current certification in CPR/First AID

During the inspection a outlet accessible to children under 5 years was observed uncovered. This was corrected by the outlet being covered with a safety cover.

During the inspection the playground was observed with a large tree limb hanging down from a tree and leaning on playground fence. Observed a sink cabinet accessible to children containing rodent traps without a safety latch on the cabinet to prevent children in care from accessing items under sink. The traps were removed during the inspection.

During the inspection a caregiver with a pending background check was accessible to children in care and counted in ratio. This was corrected by the caregiver leaving the operation and being replaced with caregiver with a cleared background check.

During the inspection it was determine that two caregivers were missing 20 required annual clock hours.

During the inspection infant bottles were observed without name labels. This was corrected during the inspection by the director printing labels and placing on bottles.
