There was one child's medication on site that expired on 3/30/25. A second child's medication was on site that had disenrolled in March.

The operation did not have a current copy of their liability insurance to review during the inspection.

The operation's fire inspection was last conducted 4/11/2024.

The operation failed to timely report to Child Care Regulation that a child was injured and required medical treatment by a health-care professional. The incident was reported 7 days after the incident occurred.

The operation did not have the original prescription box for a child's medication.

Medication for a child had an expiration date of 8/31/2022.

A caregiver was observed on their personal phone while supervising children on the playground.

There were two epipens with expiration dates of 7/2022.

One child enrolled 8/30/2022,(21 mos) did not have a copy of their immunization record on file. Two children, (5.3 yrs) was missing their MMR vaccination.

2 children had medication on site for a diagnosed allergy that did not have the original prescription.

Children were sitting in booster seats that attach to a table and the safety straps were not used. NOTE: Caregiver secured children in the seats by using the safety straps during the inspection.

A child with a food allergy emergency plan on file did not have the parent's authorization signature on the plan.

Two children with food allergy plans on file did not have an original prescription for their epi pen medication.

Two caregivers were observed sitting on the playground with their backs toward children playing on a climbing unit.

Caregiver in a young 2 year old room was observed on their personal phone while the children were napping.

There was an epi pen with an expiration date of 11/2019 on site for a child that no longer attends the operation. There was a second child with a food allergy emergency plan that had an expired epi pen from 10/2020.

Operation was previously not able to provide a current gas inspection report. Last reported inspection was 8/23/2018. The operation did not have an updated inspection performed in 2020. NOTE: An inspection had been scheduled, however, completion of the inspection has been delayed due to the inspector needing different equipment and the main gas line needing to be scheduled to be turned off for the inspection.
