Two employees no longer employed have not been inactivated. Staff 1 5/2025 & Staff 2 November 2025. CORRECTED AT INSPECTION as staff were removed at the time of inspection.

There is a play structure on the playground that has steps leading to the top. One of the steps in cracked or broken but is wrapped with duct tape. (Picture on file. )

Out of all children' s files reviewed, 1 is missing the parent signature for the needed health statement.

There is no documentation of fire drills having been done for January, February or March. (Picture of fire safety practices form taken)

There is a diaper changing mat which is used for toddlers that has a cloth cover. (Picture on file)

Playground slice structure is missing loose fill surfacing.

Facility does not have a current Health Inspection.

Reviewed staff records: Three of the 4 staff are missing training hours: Staff A missing 6 hours, Staff B missing 7 hours, Staff D missing 3 hours. Note: Staff C has completed her annual training hours.

Reviewed staff records: All 4 staff are missing training hours: Staff A missing 16 hours, Staff B missing 24 hours, Staff C missing6 hours, Staff D missing 10 hours.

Reviewed staff records: 3 out of 4 staff have not updated the shaken baby syndrome, early brain development, sudden infant death syndrome and abusive head trauma.

Reviewed staff records: all 4 have not updated the Child Abuse and Neglect training.
