During the inspection, the operation had a deficiency on the fire inspection conducted in August 2023 that had not been corrected.

A child's file was missing a physician's statement stating the child was well enough to attend school and had been examined within the last 12 months.

A contractor had access to children without an active background check on file. The citation was corrected during the inspection when the contractor left the premises.

A caregiver has access to children without an active background check. The deficiency was corrrected during the inspection when the caregiver was removed from the classroom and replaced by caregiver with an active background check.

One of the ten personnel records reviewed was missing current training in recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and understanding early childhood brain development.
