At the time of inspection, a provisional caregiver was left alone with children which violates conditions on the background.

The operation allowed 2 year old children to play with shaving cream. The can of shaving cream states to keep out of reach of children. The two year old children were observed putting the shaving cream in their mouth.

During the course of an investigation, it was determined that a caregiver placed a four-year-old child in time out for approximately 12 minutes. The child is only allowed to be in time out for four minutes.

Three out of ten caregivers lacked training in the required 6 topics.

Five personnel files lacked the pre employment affidavit.

Six out of ten children records lacked current immunizations.

During the course of an investigation, it was determined a caregiver placed a child in a restrictive device for 27 minutes.

Three infant were observed awake in restrictive devices for more than 15 minutes.

Children were observed playing outside with no drinking water present. NOTE: This was corrected when water was brought outside.

Two infants were observed sleeping in a restrictive device.

During the course of an investigation, it was determined that a caregiver throws children's food away while they are still eating.

During the course of an investigation, it was determined that a caregiver is yelling at children.

The operation lacked the following operational policies- 746.501(18)and 746.501(28).

The toddler room had windex accesible to children.

The first aid kit had multiple items that were expired. NOTE: This was corrected at inspection

The toddler room diaper changing pad was torn.

A wooden post with nails exposed was observed on the playground, posing a risk to children. Note: This was corrected when the director removed the wooden post from the playground.

Two personnel records reviewed did not have the complete 24 hours of annual training complete. One caregiver has completed 10 hours and another caregiver has completed 16 hours of annual training.

Operation placed a child younger than 12 months of age in a playard with a blanket.Two six month old infants were observed sleeping in their playard with blankets covering their body.

Based on information received from staff and children, the caregiver failed to demonstrate good judgement when she was rough in grabbing children to make them comply with her.

Through information received with this investigation, it was determined that the caregiver was rough in grabbing a child for his failure to comply with her verbal prompts.

One classroom was not in ratio as the toddler classroom was out of ratio with nine infants/toddlers to one caregiver. The specified age group was 12-18 months of age causing this classroom to be over ratio. NOTE: This was corrected at inspection with children being moved to another classroom.
