Two out of six staff files reviewed have invalid high school diplomas. Note: Provider submitted a variance request on 10/14/2025 for one of the caregivers.

Six staff files were reviewed: six files are missing pre-employment affidavit (form 2912) and one file is missing notarized affidavit (form 2985 and orientation / center - personnel personnel policies statement.

The Children's Safety Products Certification was last signed on 01/10/2024.

Two out of eight bucket seats within a kidney table are missing safety straps.

A caregiver was in charge of supervising a group of five children ages 8mo, 10mo, 10mo, 12mo and 12mo. Classroom by over ratio by one child. Note: This was corrected when one of the children left operation, leaving caregiver in charge of four infant-age children.

The most recent fire / health inspections were not available for review at the time of inspection.

Last gas leak inspection was conducted on 12/02/2022. Note: provider contacted and schedule a gas leak inspection during inspection.

The seat to a child sized toilet was broken off in a children's restroom.

The air conditioner was not maintained and was leaking water into a toddler room floor. This posed a slipping hazard for children and staff.

The emergency evacuation and relocation diagram did not inlcude two exit paths from each room that did not open directly to the outdoors.

A group of toddler children did not wash their hands before eating their afternoon snack. Note: This was addressed with the director and children were taken to wash their hands.

An infant was combined with a group of older children of varying ages during naptime in a school-aged area. There were a total of 15 children in the group and the ages of the children included toddlers, pre-k, and school aged children. The infant was 9 months of age and the oldest chlid in the group was 9 years of age.

The emergency practices form available for review was not maintained.There was no safety documentation for fire drills practiced for the month of June 2024. There was no documentation available for fire extinguishers checks, smoke detectors checks, and carbon monoxide detector checks conducted for the months of May, and June 2024.

The sign in and out log was not maintained. A total of 16 children were in care and only 13 children were signed in. Note: This was CORRECTED during the inspection as the sign in sheet was updated.

The flush handle of a children's toilet was not working.

The operational policies reviewed were missing the following information: Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); the policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

Operation did not have complete operation policies.

Operation did not have a carbon monoxide detection detector.

Transportation vehicle had one expired booster seat.

Fire extinguisher was not properly secured in transportation vehicle. Corrected at inspection when director mounted fire extinguisher.
