Two children were not buckled in during lunch time due to the bucket seats not having any seatbelts. This was corrected when a new seatbelt was found and another one ordered and the bucket seats removed until the seatbelts get put on.

A fire drill was not conducted for the months of November and December.

Several bottles were observed with the first name and no last initial. This was corrected when the bottles were labeled correctly.

A caregiver was engaged in social media activity while caring for children.

A caregiver was using a cell phone to create Tik Tok videos while caring for children.

Several rooms had peeling paint on the wall exposed to children.

A plastic container in a 4-year-old room that holds toys was damaged, leaving rough edges that could pose a risk to children. This was corrected when the container was replaced. Rain gutters with sharp edges located on the playground were exposed to children.

An incident report form was not used during an incident that involved the safety of children. Parents were informed about the incident via phone.

A child was served an identified food that was listed on the child's food allergy emergency plan.

Based on the information gathered from interviews, a teacher was heard using a yelling unpleasant type of tone with a child in the classroom. According to the information gathered during interviews the same teacher was heard humiliating a child in front of the other children.

Several purses were observed in rooms that were accessible to children. This was CORRECTED when the purses were placed in a locked cabinet or up high.

Two children's medication was found to be expired. This was corrected when the medication was set aside to return to parents at pick-up time. This was corrected when they were removed by the director to return to parents.

The monthly maintenance checklist was last recorded in May.

The gas leak test was due in March 2024, and the last one was conducted on March 6, 2022.

The incident report was shared with the parent verbally, but it was not signed within the 48-hour timeframe. The incident occurred on 10/7, and the report was signed on 10/21.

A staff was hired in March of 2023 and has been working in the operation without receiving the eligibility notification from the background check unit.

Three out of ten children were missing a special needs statement. This was corrected when the director updated the form.

The daily buildings and grounds checklist was not available for review.

The last fire inspection was conducted on 11/18/21. Fire inspections are due annually.

Based on the information gathered from interviews, a caregiver was heard using a yelling unpleasant type of tone with the children while in the classroom. The caregiver was also using a method of time-out by having the children face the wall.

A caregiver was observed in the infant room with direct access to children without receiving an eligibility letter from the background unit. This was corrected when the caregiver was removed out of the room.

Two outlet covers were observed to be cracked and one was sharp to the touch. This was corrected when the outlet covers were replaced. Chip paint was observed on the wall in two rooms. This was corrected when maintenance was called to do the repair.

The promotion of Physical Activity was not available for review in the Operational Policies. This was corrected when it was added during the inspection.

One teacher working in the infant room had an expired SIDS training. This was corrected when the director had the staff complete the training during the inspection.

Two expired medications were observed. This was corrected when the director left them out to return to the parents at pick-up.

A caregiver did not use good judgment when she allowed two-22 -month old's to stand on a two-step stool to do water play with a toy in the hand-washing sink. This resulted in a child falling and busting his lip open and had to get stitches.

There were several bottles observed in the infant room with only the first name and not the last initial. This was corrected when the last initial was added to the bottles.
