An infant did not have their safety strap fastened while on the highchair.

The roatation structure is not 12 feet apart from slide.

A medication form was missing the director's full name for the person administering the medication.

A staff was hired on November 13, 2024, was observed in the infant room without receiving the eligibility notification from the background check unit.

At the time of inspection, one medication was expired.

At the time of inspection the toddler caregiver was using the cell phone while in direct care of a group of children.

One personnel file reviewed had expired CPR and first aid training.

Two children's files did not contain health statements.

At the time of inspection the child in the high chair was not strapped in the chair. This was corrected when the child was strapped in the high chair.

One child's medication was expired at the time of inspection.

At the time of inspection the children's snack in the 18 month old classroom was served on bare table. This was corrected when the napkins were placed under the snack.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.
