There was sufficient evidence found during the DFPS investigation to show that adequate supervision was not provided when a child was injured after taking their shoes off outside.

One caregiver did not wash an infants hand after a diaper change. The infants hands were washed during the inspection.

Seven of eight infant feeding instruction forms did not have an updated parent signature.

The operation did not have a guide to first aid with the first aid kit.

Out of ten staff files reviewed, seven were missing one or more of the following required training topics: emergency preparedness, communicable diseases, administering medication, responding to food allergy emergencies, understanding building and physical premises safety, and handling hazardous materials.

The operation did not conduct a fire drill in April. This was corrected when a fire drill was conducted during the inspection and all children were able to exit in under three minutes.

During the investigation, it was determined that the operation did not notify the parent immediately after a child was injured and required medical treatment.

During the monitoring inspection it was observed that two children did not wash their hands immediately after their diaper was changed. This was corrected at inspection when hand washing corrected and further observed during the remainder of inspection.

During the monitoring inspection toddlers were observed walking around their classroom with their sippy cups. This was corrected at inspection when the sippy cups were removed from the children walking around.
