This standard was evaluated as part of the investigation and found to be deficient. A child was punished by withholding the afternoon outside play time and the child was inactive for an inappropriate amount time.

This standard was found to be out of compliance through an investigation. A caregiver had left an infant unsupervised for around 5 minutes in a bathroom.

At the inspection, 3 out of 10 personnel training files reviewed did not meet the annual training hours required by caregivers. One caregiver needs 2 hours of annual training, another caregiver needs 17 hours of for the training year and a 3rd caregiver needs 12 hours of annual training to complete their training year.

At the inspection, 1 out of 12 training records reviewed needed 1 hour of annual training to complete the training year.

At the inspection, the operational policy was reviewed and did not include the information for the promotion of indoor and outdoor physical activity.

At the inspection, the operation did not include the name and address of the alternate shelter used away from the operation

At the inspection, the training records reviewed was all on-line and no instructor led training was recorded.

This standard was evaluated as part of an investigation and found to be deficient. A caregiver inappropriately disciplined a child while in care at the operation by pulling the child's hair to get the child's attention and pushing the child back.

At the inspection, the current gas leak check is dated 2/27/2020 and is due for the 2 year check.

At the inspection, the fire extinguishers are dated June 2021 and required to be serviced.

This standard was evaluated as part of an investigation and found to be deficient. The operation left a child unsupervised on the playground.

During the investigation inspetion, a four month old infant was asleep in a crib and was swaddled. The caregiver corrected at inspection.

The caregiver failed to demonstrate competency by not counting the number of children in care before leaving the playground and as a result of the caregiver not counting, two children were left on the playground alone.

Two children were left unsupervised on the playground for 5 minutes.

10/1/21 - The fire inspection is re-cited at today's follow-up inspection. The operation has an expired fire inspection dated 6/8/21.

The loose-fill at the slide exits did not measure and the ground was exposed.
