During the investigation it was determined that a caregiver used profanity in the presence of children while struggling to keep them laying down during naptime.

During the investigation it was determined that a caregiver grabbed a child by their arm and leg, lifted the child up, then aggressively placed the child back on the nap mat. The evidence also shows the same caregiver grabbing a 2nd child roughly while in their crib during naptime.

The operation did not have a current fire inspection at time of inspection.

The caregiver did not show competency or good judgment handling the incident. The caregiver failed to notify management or person in charge of the child being injured. Reviewing video footage provided, the caregiver never got up from sitting on the ground during the incident. This included intervening prior to the injury.

The caregiver present listed observation and tlc was provided for child with head injury.

The operation did not submit background checks for staff present.

A caregiver present did not have current SIDS training and was working with children under 24 months of age.

It was determined staff present did not have Pre-service training before entering a classroom.

It was determined a staff member did not have current CPR training.

The operational policies did not include how parents can access the Texas Abuse and Neglect Hot line.

One caregiver did not have current CPR training on file.

One caregiver did not have current first aid training on file.

Out of ten files reviewed, one child did not have current immunization records available at the time of the inspection.
