The infant room was observed displaying a screen saver on the television in the classroom.

Two out of ten staff files evaluated did not have first aid training.

One out of ten child files evaluated did not have a current vaccination record on file.

Two infants had their feeding plans last updated in February.

One out of ten child files evaluated was missing an address for the listed emergency contact person.

A caregiver demonstrated poor judgement when she questioned a child regarding possible abuse rather than reporting her concerns for investigation by appropriate state agencies.

The infant crib sheets were not tight fitting, allowing the material to bunch along the sides of the mattresses and in the center if pulled. NOTE: Caregivers replaced the sheets with ones that were tight fitting during the inspection.

Two cribs in the older infant room were not labeled.

A child's immunization affidavit expired 4/6/24 and a current affidavit was not on file.

The operational policy or parent handbook did not contain a policy regarding the promotion of active play.

1 out of 10 staff training files evaluated had completed 8 out of 24 annual training hours for the evaluation period.

A child with a food allergy plan on file was missing the parent signature on the plan.

2 out of 10 child files evaluated were missing contact information for the listed emergency contact person.

The director had completed 14 out of 30 required annual training hours.

CPR training for the director had expired 12/2022.

1 out of 10 staff training files evaluated had completed 4 out of 6 annual child development training hours.

A bottle of Benadryl in the medication box had expired on 2/23. NOTE: The medication was removed from the medicine box and returned to the parent.

First aid training for the director expired 12/2022.

3 out of 10 child immunization records reviewed were missing current vaccination records.

A caregiver with a pending eligibility status was in direct care of children. NOTE: Caregiver was sent home during the inspection.

The last fire inspection was conducted on 5/10/21.

There was a Little Tykes slide on the younger playground area with a platform height of 24 inches. The surfacing underneath was astro turf on top of concrete.

The gate door entering the younger playground is loose and causing the door to not close properly.

Three infant cribs were placed adjacent to one another, preventing staff from accessing the children that were napping.

Five infants were noted sleeping in cribs. Those five cribs were not far enough apart from one another.
