A child picked up medication and put it in their mouth. The medication was in the child's mouth for approximately one minute before a caregiver removed it.

A caregiver failed to demonstrate good judgement by carrying loose medication in their pocket and dropping it. A child, then, picked up the medication and put it in their mouth.

One child was left inside a classroom for approximately 20 minutes while the remaining children were on the playground.

The operation had not exited the building in under 3 minutes for a fire drill for January, February, March and April of 2025.

During an investigation, it was found that two caregivers were sitting down on the playground preventing them from being able to intervene before a child injured themselves.

It was found during an investigation that two caregivers did not exhibit good judgement when they were observed sitting on a slide while children were present outside which led to a child touching an exterior door, causing the child to burn their hand. Additionally, the caregivers did not give appropriate first aid to a child that was injured at the operation by applying ice from the caregiver's personal cup to the child's burn.

A child was left on the playground for about 3 minutes.

There was sufficient evidence found during the investigation to show that one caregiver used prohibited punishment when yelling at children in one classroom. The caregiver is no longer employed at the operation.

There was sufficient evidence found during the investigation to show that one caregiver used prohibited punishment by slapping a child on two separate occasions, once on the head and once on the arm. The caregiver received a discipline report and training after the first incident. The caregiver was let go after the second incident and is no longer employed at the operation.

Based on information obtained during an investigation, it was determined that the childcare facility did not have Incident Reports regarding situations that placed a child at risk when a child was slapped by a caregiver on two separate occasions. The caregiver is no longer employed at the operation.

Based on information obtained during an investigation, it was determined that the childcare facility did not notify Child Care Regulation within the required timeframe regarding a situation that placed a child at risk when a child was slapped by a caregiver on two separate occasions. The caregiver is no longer employed at the operation.

Based on information obtained during an investigation, the operation self-reported via email and voice mail but did not speak with a Child Care Regulation inspector within the 48 hours which is required for reporting. This was corrected at inspection when the director informed me they would call the 24/7 hotline if they are unable to speak with a Child Care Regulation Representative.

It was found during a self reported investigation that a child was left alone in the hallway for 3 minutes. The child briefly walked into a workroom and a bathroom. This was corrected when the director wrote up the caregivers, retrained them on safety and supervision, and administration will assist them with transitions. The director stated doors of empty rooms in the hallway would remain closed to prevent children from entering them unsupervised.

The last written proof of a maintenance checklist completed was on 5/2023.

A caregiver grabbed a child by the wrist and arm as a way to direct them into help other children clean the room.

A child was left unsupervised on the playground for approximately three minutes. This was corrected at the time of the inspection when all caregivers were observed supervising in compliance with minimum standards. The caregivers were counseled and retrained on supervision. Management assists with transitions to and from the playground.

Observed children ages 20m-23 months old watching television on a phone. This was corrected when the phone was put away.

Observed one classroom who stated that they have 5 children, but there were actually 6 children in the classroom. Another caregiver had to count the children in the classroom to know how many children they had.

Two caregivers in seperate classrooms were not aware of how many children they were responsible for during nap time.

It was found that two children were left unsupervised on the outdoor playground for approximately 2-4 minutes.

Incident reports were not completed for an incident in which children were left unsupervised on the playground. The incident reports were completed during the inspection for parents to sign.

The operation did not recieve notification about the background check before having a caregiver in a classroom with children. This was corrected during the inspection when the employee was removed from the classroom.
