The caregiver failed to ensure that all parts of the child?s body were positioned/located away from the rocker prior to sitting in the seat. The caregiver did not move the child a safe distance from the rocker prior to sitting or choose a different seating area away from where the child was playing on the floor.

Two children in care did not have a health care professional statement.

The director and one caregiver who provides transportation did not have the required 2 hours of annual transportation training.

A caregiver counted in ratio had an expired pediatric CPR training.

The operation had expired Epi Pens for three (3) children in care with a diagnosed food allergy.

Three children in care with a diagnosed food allergy did not have a signed and dated food allergy emergency plan.

On two repeated inspections, infants have been observed sleeping with coverings in their safe sleep space. The director has failed to ensure that caregivers are providing an empty crib for the infants to sleep with reduced risk.

Children were observed sleeping with a covering over their bodies. Both coverings were removed at the time of inspection.

The use zone for two bike track and a rocking piece of equipment had 2-4 feet of use zone on some sides of the equipment. The minimum use zone is 6 feet.

Fire drills were not conducted in the months of October and November.

A 10 month old infant was sleeping with a blanket. An 11 month old infant was sleeping with a blanket and a pacifier attached with a clip. Both blankets and the attachment were removed at the time of inspection.

Training certificates for the complete annual training period of 2020-2021 were unavailable for 8 employees at the time of inspection.

The first aid kit in the bus used to transport children was missing scissors, a thermometer, tweezers, and cotton balls.
