An investigation found that a caregiver "popped" a child on the mouth as a form of discipline.

The vehicle that the operation uses for transportation did not have an electronic child safety alarm that was installed.

There was an employee that was present without a backgground check avaiable for review.

There was a child that was left unattended in their classroom for about 30 minutes.

There were 5 children that got out of the playground gate, and were playing in the front lawn of the operation.

The operation has not been conducting fire drills

The last fire inspection was done in July of 2023.

Two 3 year olds were left unsupervised in the restroom, which led to one child touching the other's private parts.

There were two caregivers present during the inspection who did not have eligible or active background checks. The two caregivers left during the inspection.

A child was drinking out of a water bottle when a caregiver grabbed the water bottle out of the child's mouth, resulting in the child sustaining a bruised lip from the water bottle.

Operation has not completed Children's Products Certification Form.

Ten staff training records were reviewed. One file is missing first aid/CPR training, a second file is missing emergency preparedness training certificate and a third file is missing Safe/Sleep/Shaking Baby training certificate.

Rugs in the toddler room were observed with multiple stains.

Most recent gas inspection report was not available for review at the time of inspection.

One out of ten children's records reviewed is missing immunization records.

Out of 10 staff files reviewed 10 were missing proof of education, 10 were missing completed affidavits, 8 were missing criminal history documentation, and 4 were missing photo IDs.

No feeding instuctions were available for the infants not yet ready for table foods.

Out of 10 child files reviewed 10 were missing complete emergency contact information.

Out of 10 child filed reviewed 3 were missing immunization records.

Documentation was not available to show completion of shleter/lockdown drills, and monthly checks of the smoke detector, fire extinguisher and carbon monoxide alarm.

Out of 10 staff files reviewed 5 were missing training in CPR/First aid.

One caregiver was present at the operation who did not have a current eligible background check. The caregiver left the operation.

The last fire inspection was conducted on 1-12-2019.

The last health inspection was conducted on 8-29-2019.

The operation does not have the annual product certification completes.

The last gas leak inspection was conducted on 2-15-2018.

The board of directors are not all listed on the controlling persons tab in the operation's portal.
