A 9month old was observed with a bib around the neck while sleeping in the crib. A 10month old was observed with a empty milk bottle in the crib while sleeping. This was corrected at inspection once the bib and bottle was removed from the cribs.

The most recent sanitation inspection was from 2023.

During a DFPS investigation, the young toddler classroom was out of ratio by one child.

There is a 7 year old enrolled at the operation but the maximum age group the permit allows is preschool age children.

The ceiling in the jellyfish classroom was missing tiles.

Observed a 3 month old infant in a crib with a light blankete on the infants lower body. This was corrected when the caregiver was asked to remove the blanket from the infants crib. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078

Two fire extinguishers that were present at the operation had a service date of May 2021.

While observing the first aid kit it was found that the alcohol wipes had an expiration date of 2015. This was corrected at the inspection when the director threw away the wipes.

While reviewing records it was found that the last fire inspection was done June 2, 2021.

While viewing the parent handbook it was found that the importance of active play was not present in the handbook. The last time the handbook had been updated was January 2022.

While observing infants in the kitchen and in classrooms it was found that the infants were sitting in high chairs but not strapped down. This was corrected at the inspection when the caregivers went through and strapped all the infants in the seats.

While observing the a infant classroom the caregiver was not aware of how many children she had or their age ranges.
