A 14 month old was observed eating lunch in a chair without the use of safety straps provided.

In the infant classroom, I observed a five month old child lying on the carpet, drinking a bottle that was propped on a pillow.

A caregiver was observed changing a toddler's diaper and did not wash the toddler's hands.

Two toddlers were observed standing on top of furniture in the classroom.

The transportation binder did not have medical treatment authorizations for all children being transported.

One of four personnel records was not available for review at inspection.

Two out of six children files evaluated needed an updated immunization record.

Seven out of 10 infants did not have an updated feeding instrutions signed by a parent.

During the follow up inspection, a caregiver was observed mopping the restroom while caring for 23 children ages 3-5 years old at nap time. This was corrected at the time of the inspection when the caregiver stopped mopping.

Based on the information obtained during the investigation, there was sufficient evidence to support that a child in care was kicked.

It was found that one classroom did not have the sleeping cots labeled with the child's name. Note: This was corrected at inspection when the cots were labeled.

It was found that two infants (3 months old) did not have parent signature for the feeding instruction for the month of October.

Based on the information obtained during the investigation, three caregivers tested positive for coronavirus and the illness was not reported to licensing.
