During the monitoring inspection it was observed that a person present did not have a background check to be present and around the children. This person was in the gym area, while the children in care were also present. Background check was ran while Licensing was present at the operation and the subject will not present around the children in care until operation has clearance from the Centralized Background Check Unit.

During the inspection staff files were reviewed and it was found that an employee who had been employed with the operation for over a year did not have their 15 completed annual training hours.

During the monitoring inspection it was found that three children enrolled had a food allergy, however none of them had a completed emergency form from a physician.

During the inspection the operational policies were missing components, parental notification, expulsion, and promoting outdoor,

During the inspection, expired ointment, and medications were observed in the first aid kit. This was corrected during the inspection when the provider removed and threw the medications and ointment away.

A staff member was present at the operation during operating hours, with access to children without a cleared background.

During the inspection, 2 of the operation's transportation vehicles did not have first aid kits. One vehicle had expired medications.This was corrected when the director removed all expired medications and threw them away.

During the inspection one staff did not have the required training for transporting.

During the inspection, the first aid kit was incomplete. There were no scissors, cotton balls, antiseptic wipes or a thermometer. This was corrected during the inspection when the items were added to first aid kit.

The caregiver failed to notify the parent that an incident had occurred. The parent was not given a written incident report until two days after the incident.

During the investigation it was found that caregivers are yelling at children as a form of discipline and lack training of discipline methods.

The operation did not conduct a fire drill in May, June, or July of 2022.

Two children with diagnosed food allergies did not have a food allergy emergency plan.

During an investigation it was found the operation failed to notify the parent of the victim when the child was left unattended on the daycare vehicle

During an investigation it was found the operation failed to notify Regulation/Licensing within the required time frame when a child in care was left unattended inside a daycare vehicle

During an investigation there was sufficient evidence to support a child was neglected when two caregivers left a child unattended on the daycare vehicle without adult supervision for approximately 25 minutes.
