During the inspection only 3 personnel records were available out of 6 staff employed.

During the walk through the infants were seen not strapped in to the table bucket seats as required by the manufacturer for safety.

During the inspection an infants hands were not washed after a diaper change was completed.

During the inspection infant diaper changes were observed. The infant's hands were not cleaned after having their diaper changed.

During the insepction a caregiver was seen making a personal call on their cell phone while caring for children.

This standard was found to be out of compliance through an investigation when it was determined an infant was left inside a restrictive device for more than 15 minutes.

A copy of the operations policies were reviewed and were missing the parents rights information.

The operation added to the outdoor space on their playground without informing CCR. This was corrected at the inspection when the addition was reviewed.

During the inspection 10 children's records were reviewed. 7 of the records did not have the address listed for the emergency contact.

Staff were questioned on active play routine with bad weather days, all staff stated they get active play by having the children play in their centers. Children could not go outside during inspection due to poor weather conditions.

During the inspection children's immunizations were observed. Out of 10 records 8 were out of date for their shot record.

Only 11 hours of annual training were available for review during the inspection.

When reviewing personnel files all employees reviewed who had been employed for at least 12 months were missing the required training topics.

The operation had not reviewed and re certified their Children's Products Certification for the year. This was corrected at inspection when a walk through of the building and certification was completed.

During the inspection a caregiver was observed yelling at a child.

The most recent inspection report from the application inspection was not posted during this inspection. This was corrected at the inspection when the operation printed it off and posted the form.
