There were no current infant feeding instructions for five infants.

Employment affidavit was not available for review for 2 out of 8 employee files reviewed.

Caregiver pulled a mat from under a child while sleeping.

Caregiver walked out of the classroom leaving the room without supervision.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

A fire drill was not completed for the month of August and September as per lack of documentation. This information was confirmed by Director of operation.

Based on evidence gathered during the investigation, the operation was deficient in providing the supervision needed to intervene timely during an incident between children.

In the course of the investigation, it was confirmed that operation is allowing children to use electronic devices for screen activities not related to the activity plans of the operation.

A staff member pending background check eligibility and with access to children was observed present at the operation. NOTE: Individual left the operation during the inspection. Director confirmed individual will not return until eligibility letter is received.

It was confirmed by Director that the operation does not allow parents to visit the child-care center as specified in this standard.

Children's hands were not washed before they were provided with their bottle.

The following trainings were not available for review for Director of operation. Training year September 2021-2022. -Handling, storing, disposing of hazardous materials -Food and Allergic reactions -Administering medication -Transportation

Sanitation inspection was last completed in 2021. Inspection was not completed within the 12 month time frame.

The operation has a capacity of 14 infants. The operation had 23 infants present during the inspection. The limit of infants per classroom's space measurements was exceeded. NOTE: Children were picked up by their parents during the inspection. A request to increase infant capacity was submitted by Director of operation.

Child/Caregiver Ratio was reduced in two infant classrooms. Classroom #1 had 7 infants with one caregiver with a specified age group of 17 months. Classroom #2 had 5 infants with one caregiver with a specified age group of 7 months old. NOTE: This was corrected during inspection as a second caregiver was added to each classroom.

Fire inspection was not available for review. Last inspection was available was of 1/27/2020.

Interest centers were not present in the Pre-Kindergarten classroom as observed during inspection.

Carbon monoxide detector was not present at the operation.

Infant daily report is not being completed in one of the infant classrooms.

Out of the staff files reviewed, 2 were lacking proof of a completed TB test.

2 caregiver had no verification of having received the remaining 16 hours of pre-service training.

Staff records showed that 2 staff were missing affidavits.

There is a caregiver is who lacking 4 hours out of the required 24 needed for the year being reviewed. (10/8/19--10/7/2020)

There is a caregiver who is currently lacking training in child maltreatment.

There is a caregiver whoh did not have any current training in SIDS/Shaken baby Syndrome.
