The primary caregiver was missing training for the previous training year in the following topics: Recognizing and preventing shaken baby syndrome, Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS), Understanding early childhood brain development.

The carbon monoxide detector, smoke detector, and fire extinguisher were not being checked monthly. In February 2024, this equipment was not checked by the primary caregiver.

The primary caregiver has 24 hours of the required 30 hours of annual training. The primary caregiver does not have the following annual required trainings: emergency preparedness, administering medication, understanding building and physical premises safety, and handling storing, and disposing of hazardous materials.

The primary caregiver does not have a current CPR/First Aid.

The operation was observed using absorbent sheet for diaper changing.
