An infant was observed sleeping on an elevating medical pillow with straps due to having reflux. The infant caregivers stated that two infants use these pillows and that they also use them in their cribs. The infants had doctor's notes but no Infant Sleep Exception forms on file.

Three classes were combined outside with the youngest child being fourteen months old and the oldest being thirty-five months. This was corrected during the inspection when the infant class was brought inside until the other two classes were done on the playground.

Three infants were observed sleeping on boppy pillows and elevating pillows on the floor. This was corrected during the inspection when the infants were moved to their cribs.

During the investigation, there was sufficient evidence found to determine that caregivers were aware that children were climbing on shelves but did not intervene, which resulted in a child getting injured.

The infants' mood and summary of daily activities were not being recorded every day in the Learning Genius app. This was corrected during inspection when the caregivers were informed that those sections needed to be recorded daily and the caregivers started adding them.

Out of ten staff files reviewed, one's first aid and CPR certification had expired and one did not have first aid and CPR certification.

Out of the ten staff files reviewed, two were missing required training in the following topics: building and physical premises safety, handling hazardous materials, and emergency preparedness. This was corrected at inspection as the evaluation period had already ended, and the caregivers had obtained this training for the next evaluation period.

A caregiver was heard telling a child, "If you don't listen, you will not go outside."

An 8 month old infant was left in their classroom for 1 minute and 14 seconds during a fire drill.

There were 2 infants in care that did not have updated infant feeding instructions on file.

There were 6 infants (12-14 months) observed in their classroom with one caregiver. Another caregiver was sent to the classroom to bring the ratio into compliance.

During the inspection it was observed that a pre k class came inside, after playing outside, and did not wash their hands before they sat at their tables and began playing in centers.

During the investigation inspection it was found that the two caregivers who's files were reviewed did not have any documented training hours available for review.

During the investigation inspection it was observed that a 3 month old infant was placed on their stomach to sleep.

During the investigation inspection it was found that both caregivers left the classroom after an incident between two children had occurred, leaving the classroom of children without a caregiver for a few seconds.

An incident occurred between two children in care where the two children began to fight and hit one another. One of the children was hit with a metal bell before a caregiver intervened to stop the children from fighting. One of the children required stitches due to this incident.

Observed the same toy being place in mouth by different children multiple times without being set aside to sanitize. Toys were placed back in toy bin with clean toys. This was corrected by the toys being taken out and set aside to be sanitized.
