A naptime is not being proivded for children 4 years of age.

A caregiver did not have current training in Pediatric first aid with rescue breathing prior to having unsupervised access to children in care.

The operation has not designated a qualified director since 11/10/2022.

Two classrooms were observed with black mold on a wall located behind children's furniture. A third classroom was observed with stains that looked like mold on a wall located behind construction paper. The color of the stain in the third classroom was black, yellow, and green. External walls located on the North and South side of the building require maintenance. Part of a wall was bulging, and another section of the wall was cracked with an opening large enough where rain could seep through. The air conditioner vents in the classrooms were observed with dust and debris. Carpets in classrooms require maintenance. Carpets were observed with black stains.

A plastic border for the mulch was broken from a corner in the outdoor playground. This posed a safety hazard for children as the broken piece had sharp edges and is accessible to children. Note: Duct tape was used to cover the broken area temporarily.

Screen time was being provided to a group of children during lunch time.

Caregivers demonstrated poor judgment when they allowed children to run around the classroom and not intervene or redirect them. This created a situation in which a child fell and injured themself.

Operation staff did not contact emergency medical services immediately after a child was severely injured. The parent of the child was contacted and was provided with the choice of picking up the child or contact emergency medical services.

A caregiver did not have current training in pediatric first aid with rescue breathing and choking. The expiration date on the certificate of training reviewed was 9/21/2021.

A caregiver did not have current training in pediatric cardiopulmonary resuscitation (CPR). The expiration date on the certificate of training reviewed was 9/21/2021.

Based on the information obtained during the investigation, it was determined that a caregiver was conducting janitorial duties while caring for a group of children.
