A caregiver released a child in care to an unauthorized person.

A caregiver fed an infant in care a bottle not intended for said infant. The infant required medical treatment.

The caregiver grabbed the child and put him on the nap mat roughly.

A caregiver pulled a child up by his arms and carried him resulting in nursemaids.

A caregiver did not show good judgement when they pulled a child up by his arms and as a result got nursemaids.

Loose fill in the prek and school age equipment area was compacted not allowing correct loose fill height. Corrected at inspection when operation informed all caregivers the outdoor equipment is not to be used until loose fill at proper height to allow shock absorbing and marks on posts.

One fire extingusher in bus called boswell and one in blue van were not serviced since 2022.

Four infants did not have updated feeding sheets available for review. This was corrected at inspection when parents emailed updated information at time of inspection.

One child is missing admission information for transportation, field trips, and water activites.

One infant under 12 months old was observed sleeping in an infant swing. Child was removed from swing at time of inspection and moved to a crib.

This standard was evaluated during a DFPS investigation and found to be not compliant. One caregiver had access to children without a current eligibility result.

The director stated during the investigation that she had only notified the parents in the classroom where close contact was made with the infected individuals and not all parents. Licensing observed this was the case on the operations electronic messaging system. The system showed that only certain parents were notified and not the parent of each child attending the child-care center as required by the standard.
