Two children were missing a medication listed on their food allergy emergency plan.

The first aid kit had expired eye wash inside of it. The staff removed this during inspection from the first aid kit.

One staff did not have completed SIDS/Shaken Baby training.

One staff did not have current CPR training that was completed in person.

Two classrooms had garbage accessible to children. This was corrected at inspection when the staff removed one trash can and closed the lid on the second one.

One infant was observed asleep in a crib with a sound machine hanging inside of the crib. The caregiver removed the sound machine from the crib during inspection.

The operation now has two birds at the center and did not notify parents in writing.

The operation did not have a sanitation inspection done within the last 12 months.

A caregiver inappropriately grabbed a child and moved them from one area of the classroom to another.

The operation did not provide the Licensing Incident/Illness report form, or any written injury/illness form to parents of children that were injured in care.

The operation did not have a current annual fire inspection as the last one conducted was in 2022.

The outdoor play area had two sprinkler houses in the ground where sprinkler wires and access are stored that were uncovered. One of them also had an exposed wire. This was corrected as the operation covered these with appropriate covers purchased.

A shelf was observed blocking the emergency exit in a classroom.

One child was unsupervised allowing the child to leave out of the front door.

The operation has not been conducting monthly fire drills.

One caregiver in a classroom did not know how many children she was responsible for. This was corrected at inspection when the caregiver was made aware of the correct number of children present in the classroom.

The operation did not notify CCR of a situation that placed children at risk while in care.

One caregiver was present in a classroom with children without an eligible or active background check. The caregiver was sent home during the inspection.

During an investigation it was found that one current caregiver, and one prior caregiver yelled at children in care.
