The operation did not have a playground checklist for the past month at review.

Two out of ten staff training records did not have the required trainings.

A two year old child was left on the playground for approximately ten seconds. *An admin was outside on the other side of the operation and went to the child. *

During the investigation, multiple caregivers noted a child crying on the playground, but did not assess the child to determine the cause. The child had an injury that required medical treatment that caused the crying.

During the investigation, it was stated the victim was seen playing on play structure over 32 inches.

At time of inspection, a caregiver stated they had seven children when they had eight. This was corrected at inspection.

A caregiver did not interact with a child in a positive manner when they pulled a child's hair to prevent them from crossing the street.

At time of incindet a caregiver did not have a current CPR/First Aid certification.

At time of inspection, infant monthly feeding sheets were last updated in June 2024.

A toddler was found unsupervised in operations playground on 4.30.24.

At time of inspection two diaper changing stations were observed to have debris. This was corrected at inspection.

A toddler was found unsupervised in operations parking lot on 2.5.24.

At time of inspection operation did not have emergency preparedness plan included in parent handbook. At time of inspection, operational policies did not include months open, immunizations, promotion of indoor/outdoor activity and gang free zone.

At time of inspection an infant child was observed sitting in a restrictive device and was not strapped in. This was corrected at inspection.

At time of inspection two diaper changing tables were observed with debris.

Based on the information gathered a caregiver slapped their own child while in care.

Based on the information gathered a caregiver yelled at their own child while in care.

Based on the information provided the first aid card was expired for the staff member involved in the incident where a child was injured and required medical treatment.

Based on the information gathered a caregiver struck a child with a long plastic tube while the class was playing on the playground.

Based on the information gathered another caregiver that was not qualified was left alone more than occasion with a group of children.

Based on the information gathered a situation that placed a child at risk was not reported within the required time frame.

At the time of inspection, there was two children in the high table in the infant room without their seat belts fastened around them. This was corrected when the caregiver fastened the seatbelt around the children.

Based on the information gathered the caregiver did not sanitize the changing table before placing another child on the changing table.

Based on the information gathered the operation failed to report an incident that placed a child at risk.

Based on the information gathered there was a soiled diaper accessible to a child on the changing table.

Based on the information gathered a child was left unattended on the changing table.

Based on the information gathered the caregiver had not obtained a high school diploma or GED, and was alone with two children in the classroom.

Based on the information gathered the first aid kit was accessible to the children in a classroom that resulted in a child having access to open the kit.

Based on the information gathered, it was determined that the child was not supervised when the child opened the first aid kit and removed the scissors, and placed the scissors in the child's mouth. The child walked out to the playground and slid down the slide with the scissor still in the child's mouth before another caregiver on another playground noticed the scissors. Note: The rubber gloves and safety pins were also found on the playground from the same first aid kit.

At the time of inspection there was one infant in the 12 to 17 month old classroom with their head covered. THis was corrected when the child's head was uncovered.

At the time of inspection the changing table in the 12 to 18 month was not disinfected after diaper change.

The infant gate was broken in the 12 to 18 month old classroom. This was corrected when the gate was replaced with a new gate. The floors in the toddler classrooms were dirty.

At the time of inspection the fire exit was blocked in the two year old classroom.

At the time of inspection the caregivers in direct care of children were using their cell phones.

At the time of inspection the caregivers did not know who was in charge of the operation. This was corrected during the inspection.

At the time of inspection the child's hands were not washed with soap. This was corrected when the caregiver, washed the child's hand with soap.

At the time of inspection the caregiver in the toddler room did not sanitize the diaper changing table. This was corrected when the caregiver sanitized the diaper changing table.

It was determined that a child was left unsupervised in the classroom.

Three carpets throughout the building were dirty and need to be cleaned. This was corrected when the carpets were removed from the classrooms.

The diaper changing mat in the 12 to 18 month old classroom was ripped and could no longer be disinfected. This was corrected when the changing mat was replaced with a new mat.

One infant feeding schedule was expired at the time of inspection.

Two shelves were not secured to the wall in the toddler classroom. This was corrected when the shelves were removed from the classroom.

At the time of inspection there was a fan accessible to the children in care. This was corrected when the fan was removed for the classroom.

At the time of inspection there were very few toys in the toddler classroom. This was corrected when the toys were returned to the classroom.

At the time of inspection, there were not any napkins under the snack served in the four year old classroom. This was corrected when the napkins were placed under the snack being served.

Based on the information gathered the caregiver in the 12 to 17 month old classroom left a child on the changing table while walking across the classroom to the trash can.

Based on the information gathered one shelf was not secured in the 18 to 24 month old classroom, when it fell on top of a child.

At the time of inspection there were two infants napping in restrictive devices. This was corrected when the children were placed in the crib to nap.

The paint behind the toilet in the three year old classroom was peeling.

One infant feeding schedule was expired.

Two caregivers had expired SIDS and Shaken Baby Training.

Two nap mats were torn at the time of inspection.

One caregiver did not complete the 24 hours of preservice hours within the first 90 days of employment.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.

At the time of inspection the caregiver in the infant room was using the cell phone while in direct care of a group of children. This was corrected when the caregiver put the phone away.

At the tie of inspection one caregiver did not have the first eight hours of pre-service training before being placed in direct care of a group of children.

One caregiver did not have first aid certification before being placed in direct care of a group of children.

One infant feeding schedule was out of date at the time of inspection.

At the time of inspection there were not any current activity plans posted as the center is changing the curriculum.

The soapy water and clear water were not being used to disinfect the changing tables with the disinfecting solutions at the time of inspection.

There was a small rip in the changing table in the infant room with the foam protruding from the plastic.
