A caregiver was observed feeding two toddlers from the same container of yogurt. Individual servings or containers of yogurt were not used for feeding the children. A caregiver in the infant room served children their snack on a bare table.

Screen time was provided for children in a toddler room during the afternoon snack time. A child under 24 months of age was also present iin the room. The children were watching music videos on a tablet.

A group of school-aged children were observed resting on the floor of the front lobby area. The lobby area is not designated for children or licensed space.

A metal playhouse was observed with peeling paint in one of the outdoor playground areas.

Nine staff files were reviewed. One files is missing certification for management training courses and a second file is missing 4hrs of pre-service training and notarized affidavit.

A 3 month old was observed sleeping in a swing in the infant room (red room). *This was corrected at inspection when 3month old was removed from restrictive device.

18 Children were observed napping in the gray room with little to no space inbetween mats, room was also observed dark with limited walking visability. One caregiver was present.

2 play-yards were observed with a mat that had a large gap between the mat and the edge of the play-yard. 1 of the mats had a gap of 3inches. *This was corrected at inspection when the mat was snapped to the base of the play-yard.

Children's Products Certification was last signed on 07/2022. Note: This was corrected at inspection when provider completed and signed the certification.

Prek play structure is surrounded by 3 inches of pea gravel and toddler play structure is surrounded by 4-5 inches of pea gravel.

One out of ten children's files reviewed has a diagnosed food allergy and missing an emergency food allergy plan. Note: This was corrected at inspection when provider confirmed child was withdrawn from operation and updated file.

As of 09/2022 operation removed the school-age room in the south building. Room is no longer designated for children. Note; This was corrected at inspection when provider agreed to remove room from licensed space and updating operation's total capacity.

A child was admnistered 2mL of a medication, while dosing chart indicates 1.25mL should be administered based on the child's age.

One out of eight staff files reviewed has an invalid high school diploma. Note: This was corrected at inspection when provider submitted a variance to allow caregiver time to complete GED.

Four out of eight staff trainings records are missing certificates for trainings on the topics of Transportation, Communicable Diseases, Food Allergies and Medication Administration.

Operation consist of three separate building; there is a carbon monoxide detector in the building that has the kitchen, other building in use does not have a carbon monoxide detector.

Operation has two play structure with a height of 20 inches, there is 2-5in of pea gravel surrounding the structures.

Four out of five staff training records reviewed are missing trainings on hazardous materials, administering medications, building and physical premises safety and communicable diseases. One file is missing transportation training.
