During the follow up inspection, a school age child was found unsupervised.

During the follow up inspection, an infant child was observed walking while drinking from their bottle.

During the inspection, a child's allergy information from was not posted in the classroom or in the food preperation area. The operation later corrected this during the inpsection.

During the inspection, a food allergy plan for a child was signed and on file but the operation could not comply with the plan as they did not have epi-pen required by the plan.

In two classrooms and a children's bathroom, Lysol cleaning products were found within reach of children under the age of five while in care.

Mutliple caregiver records do not meet the requirement for 24 hours of annual traning.

Two caregiver files did not have current food allergy traning, medication administration traning or handling, disposing or storing hazardous materials traning

A caregiver did not sanitize the diaper changing area after changing a diaper.

7 children did not have an address for their emergency contact.

No playground maintenance checklist were available for review.

A caregiver was observed not washing their hands after changing a diaper.

A child was observed not having their hands washed after their diaper being changed.

The Keeping Children Safe poster was not posted for parents to review.

Two children did not have a health care provider statement in their files.

A safe sleep policy was not found in the operational policies.

A child with a diagnosed allergy was in care without a food allergy emergency plan submitted.

Caregiver and director training files were incomplete and lacking hours.

The most recent inspection form was not posted for parent's to review.

Four caregivers did not confirm they graduated from high school, provided a high school diploma, or transcript.

Three caregivers did not have an employment affidavit completed.

Promotion of physical activity was not found in the operational policy.

Three children enrolled into care did not have a health statement on file for review.

Upon review of children's files, two children did not have their most current immunization records available for review.

The two infants that were still receiving breastmilk and/or formula did not have current infant feeding instructions present at the time to review.

The operation had no conducted any safety drills since 9/2021.

The smoke detector in the 3 year olds classroom was beeping due to a low battery at the time of inspection. There was no documentation to ensure that the smoke detector had been tested in the last month.

At the time of inspection, the center director was lacking 11 hours of training from the previous full year.

At the time of inspection, the rubber mulch observed on the playground was not at proper depth; as the markings on the poles for depth measurement were exposed.

The operation's last documented fire inspection by the City of Allen was on 4/3/19.

The fire extinguisher present in the school's van that's used for transporting didn't have a tag to indicate that it had been serviced by a fire marshal.

At the time of inspection, the caregivers reviewed only had 5 hours of training from the previous year.

At the time of inspection, 3 children did not have their most current immunization records or health statements on file.

Five of the seven vehicles used for transporting children were missing fire extinguishers.

Seven of the ten personnel records reviewed were missing annual training in emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations; administering medication; preventing and responding to emergencies due to food or an allergic reaction; understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and handling, storing, and disposing of hazardous materials.

The depth of the surfacing at the slide exits, in front of the climbing wall and entrance points to the playground structure varied in depth from less than one inch to 5 inches.

Seven of the seven vehicles used to transport children were missing emergency medical transport and treatment authorization forms for the children being transported.

Seven of the seven vehicles uses for transportation were missing parent's names and telephone numbers and emergency telephone numbers for each child being transported.

The bottles for the infants were not labelled.

One of the seven drivers for the operation was missing a copy of their current driver's license in their personnel file. This was corrected at inspection when the director obtained a copy of the driver's license.

Five of the seven vehicles used for transporting children were missing first aid kits.

A driver for the operation did not have a background check and FBI fingerprint clearance.

Two infants were missing feeding schedules.

The infant and toddler teachers were missing annual training in recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and understanding early childhood brain development.
