The operation had not completed a fire drill in under 3 for the previous month.

The television was on in a classroom while a child under 2 years old was present.

The last time fire extinguishers were serviced in transportation vehicles was over 12 months ago from date of monitor inspection.

The center did not have the benefits of physical and outdoor play in the operational policy.

The booster seats that were being used expired between 2012-2014. The booster seats were removed immediately and corrected at inspection.

A step was broken on a ladder used to gain access to a slide on playground equipment.

A child's admission form stated they wer allergic to fish and shell fish. A completed food allergy plan was not in the child's file.

The director observed the caregivers in the hallway not supervising the children.

Three caregivers were observed out of their classrooms in the hallway and the classrooms were left unsupervised.

The parent handbook was lacking the criteria the center will use to define extreme weather.

Two first aid kits evaluated were observed to have band aid wrappers and old medical tape.

Children's handwashing sink was being used to wash children's training cups.

Not all cots were observed not to be labled in all 3 classrooms observed during walkthrough.

Three out of six children's records reviewed did not have an address for the emergency contact person. Corrected at Inspection: The addresses were added to the child admission form during the inspection.

The fire inspection was last conducted on 12/20/2021, and it has been over a year since the last fire inspection.

The toddler classroom had a total 12 children with one caregiver and a specified age group of 2 years old.

A caregiver present in a classroom and counted in ratio did not have an active background check under the operation. Corrected at inspection: The caregiver was sent home.

The transportation training for one of the drivers expired on 01/01/2023.

Three out of six child admission forms reviewed did not contain the address or phone number of the emergency medical physician or medical care facility. Corrected at inspection: The child admission form was updated with the phone number and address of the emergency care facility.

The black plastic underneath the pea gravel has started to surface and it causes a tripping hazard.

The loose fill in the playground surrounding a 4ft slide and rocking equipment was not six inches.

The school age room was observed watching a movie during the naptime/rest time. Corrected at inspection: The movie was turned off during my inspection.

The Pre-K and School classroom were dark enough that the eyes had to adjust.

The center last validated the employee list on 02/14/2022. Corrected at inspection: The center validated the list during the inspection.

The center dismissed a parent for not signing up for the summer session, but no other parent was asked to sign up for a summer session. The operational policy does not include a separate enrollment for the summer session.

The center's smoke detection system was last inspected August of 2020, and has not been re-tested within one year.

The fire inspection was last done on 08/19/20.

The toddler classroom was doing a screen activity and the yongest child in the classroom is 19 months old. Corrected at inspection: The television was turned off.

There was no notification posted notifying the parents of where the children would be going on the field trip or when they would return from the field trip. Corrected at inspection: The center posted a notice to the parents with all the required information.

A rocking car in the playground did not have the required 6 inches of loose fill.

The drivers nor the director had a valid transportation training.

Four out of five staff members did not complete the required 24 hours of annual training.

The fire extinguisher observed in the transportation van was last serviced October 2019.

Caregivers did not wear any easily identifiable items when they went out to the park on a field trip.

The children did not wear any item that identified what center they belong too, when they went to the park on a field trip. Corrected at inspection: The center has tags with lanyards with the centers name and phone number.

The Director only completed 8 hours of 30 hours of their annual training hours.

The transportation van's registration sticker expired on 09/2020.
