The Liability Insurance Expired 11/18/25.

One child's epi-pen expired 09/2024.

The Annual health Inspection expired 09/26/25.

Staff and children's records were unavailable for review during the inspection.

Feeding seats did not have safety straps.

There were 3 staff who does not have the required 24 hours training.

The operation was cited for not able to provide a copy of current fire inspection. The director informed the management of the church, they will need to schedule a new inspection with the fire department.

There was no documentation that a fire inspection has been done within the last year.

There was no proof of liability coverage at the operation.

There is no fenced outdorr activity space for children under 3 years old. A waiver/variance request must be sent with a plan by the compliance date if it is not possible to have the fence by the compliance date.

I evaluated 10 children's records during the inspection and noted that one of those did not have a health statement on file. It was also noted that the health statements signed by parents did not have the name and address of the health care professional who had examined the child within the last year.
