Three child files reviewed did not have the required health statement signed by a health care professional and the health statement signed by the parent had expired.

The children's files were lacking health statements.

Two personnel files were lacking documentation for Pediatric First Aid and CPR. The Director stated this training has been completed.

Two personnel records were lacking documentation for educational requirements.

The three personnel records were lackng the required affidavit.

The emergency relocation diagram was lacking documentation of the designated location inside where children and caregivers take shelter from threatening weather.

The following safety hazards were observed: Cords were observed in the infant and toddler rooms. The cords were accessible to the children in care. The playground had exposed PVC piping along the wall which is accessible to the children in care. The fencing had two areas with a large gap that a child could gain access. Pictures were taken.
