2 of 8 child files evaluated had admission forms indicating the child had been examined in the past year by a healthcare professional and that a health statement would be provided within 12 months of admission but there was not documentation in the files that the health statement had been provided within the 12 months.

One caregiver had first aid/cpr that expired at the end of November 2024. Their renewal has not been completed yet,

A blue dome on the playground was observed to be peeling in several spots, Note: Photos were taken.

Information gathered in the investigation did not substantiate that a child had to sit out from playing outside for twenty-five minutes for talking. Information gathered in the investigation substantiated that children have had to sit out from playing outside for short periods of time for behavior that occurred inside as a consequence for behavior.

The operation did not verify through their online CCR account the number of employees who left employment in 2023 by January 15th as required.

The required documentation to support a caregiver who transports children completed transportation safety training was not available during the inspection. *Note* The original certificate was not able to be located so the caregiver completed the training during the inspection.

One of the vehicle's used for transportation had an electonic safety alarm that was not working.

Two nap mats children were sleeping on were observed to be ripped and torn. Note: Photos were taken. This was corrected during the inspection when they were removed from the classroom to dispose of.

1 in 8 children's records evaluated did not have a record of current immunization requirements met.

4 of 10 children's records evaluated were missing addresses for emergency contacts listed.

The picnic table a child fell off of when an injury occurred was not appropriate for the child's age.

Based on information obtained from DFPS staff did not account for all children when unloading the bus to ensure that the bus was empty before parking the bus.

The parent was not notified immediately that the child was left on a bus unattended.

Based on information obtained from DFPS a child was left unattended on a bus for several hours.

Based on the findings from a DFPS investigation there was a finding of neglectful supervision in regards to a child who was left unattended on a vehicle.

Based on information received from DFPS staff did not demonstrate competency or good judgement in response to this incident. Staff did not seek medical treatment for the child. Staff also failed to notify someone at the elementary school when the child was dropped off what transpired; this would have allowed elementary school staff to be prepared for anything the child may have needed based on what occurred.

The operational policies need updated to reflect the required components of the promotion of indoor and outdoor physical activity section.
