2 caregivers that were trained on face to name procedures in August of 2021 did not demonstrate competency when they did not follow the face to name procedures that were taught during the training as a result a child was unaccounted for during a transition time back to the classroom.

A 4 year old child was left in a classroom bathroom and unsupervised for approximately 3 minutes when the class went outside to play.

A child that attends the daycare tested positive for COVID-19. The operation did not notify licensing of the positive case.
