E. Chapa Head Start Campus

Cleaning supplies were observed under a children's sink in four classrooms. The cleaning supplies included several spray bottles of disinfectant, a bottle of hand sanitizer, a bottle of Fabuloso, and a can of shaving cream. This posed a safety hazard as the door to the cabinet did not have a safety latch and the items were accessible to children. Note: This was CORRECTED during the inspection when the items were removed and stored in an area out o children's reach.

The operation did not immediately contact Emergency Medical Services for a child showing serious signs of an illness that required hospitalization.

The operation did not share a copy of the incident report with the parent within the required timeframe.

The sign in and out log was not current. There were children present who were not signed in and there were no initials of the teachers/parents on the children signed in. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

There was no soap in the hand washing sink area. There were children waiting to wash their hands after outdoor play but had to wait until soap was placed in the area. Corrected at Inspection: Soap was placed in the hand washing sink area.

There were 4 mats used by the children that were torn. There were several mats not labeled with the children's names. Corrected at Inspection: The mats were replaced with new ones and were labeled with the children's name.

There was a tumbler/cup with a liquid beverage inside that belonged to a teacher that was left in the hand washing sink area used by the children. Corrected at Inspection: The cup was removed during the inspection.

There are red ants in the outdoor play area. There was trash on the outdoor play area. Note: The trash was picked up during the inspection.
