One caregiver's CPR expired 09/13/24.

One caregiver did not have the required 24 hoursof required training.

The operation did not have the required Gas Inspection. Gas stove was located in the kitchen area.

The operational policies did not address the fish in the tank which are at the operation.

The operational policies did not address suspension and expulsion of children as required.

The operation had no proof that their liability insurance was current.

There was no place for the actual start date of the child in the children's folder as required.

The operational policies must include promotion of indoor and outdoor physical activity that is consistent with the standards relating to developmental activities and your policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

The operational policies did not include procedures for parents to review a copy of the child-care center's most recent Licensing inspection report and how the parent may access the minimum standards online;

Instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website

Your operational policies must include information on vaccine-preventable diseases for employees (whether they are required or not).

The operational policies did not include your policies for applying, as needed, insect repellent and sunscreen, including what types will be used if you provide these.

The operational policies did not address the requirements of which childen need vision and hearing screening and just states "mandatory for certain age children". This needs to be explained this is for children who are 4 years old by Sept 1 of the year.
