Through an investigation it was found that a child was able to reach hand sanitizer. Child put hand sanitizer in their eyes and had to seek medical attention.

Caregiver's supervising did not use good judgement when a child was allowed to play on the shelf of the changing table which resulted in the child knocking over a trashcan and bottle warmer (on to them selves). Operation did not use good judgment in contacting a parent from a personal phone instead of the operation's phone.

The caregiver in the 3-4 year old classroom during nap time was laying down, covered up, and not supervising the children during nap time. She was the only caregiver in the room.

There were infants sleeping in restrictive equipment.

The loose fill material was less than the required 6 inches under the swings and climbing structure.

There was cleaning supplies on a table accessible to the children.

The climbing structure and swing set are made of wood. There are places on both that are weather and are now creating a hazard. The swing set has some sharp pieces of wood that need to be trimmed and the structure has rope that is starting to rot.

There was a 2 month old infant observed to be swaddled and asleep on a blanket on the floor.

There was a classroom that had 16 children with one caregiver and the specified age group was 18-23 months old and there were two infants in the classroom.
