Supervision was not appropriate, which led to a child exiting the porch area and running into the road before the caregiver could get the child.

Based on information gathered and video evidence, children were playing on the porch, which is not licensed space.

Operation allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility.

The employee list has not been validated in the past three months.

There are two areas in the outdoor area where the lattice has an opening larger than six inches.

Based on information gathered, all parents were not notified immediately that their child may have been placed at risk when a child brought a pocket knife to the operation.

One child's record reviewed did not have the complete designated contact information on the admission form at the time of inspection.

First Aid Kit in classroom 2 was missing the following items: Cotton Balls, Disposable Gloves, and Band aids. NOTE: This was corrected at inspection with those items being placed in the first aid kit.
