Three of eight children records reviewed did not have a health statement from a health care professional on file for review during the time of inspection.

Promotion of physical activity needs to be added to the operational policies to include the follow: 1. The duration of physical activity at your operation, both indoor and outdoor. 2. The type of physical activity (structured and unstructured) that children may engage in at your operation. 3. Each setting in which your physical activity program will take place 4. The criteria you will you to determine when extreme conditions pose a significant health risk that prohibits or limits outdoor play. 5. A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

An infant was observed sleeping in a crip with a pacifier attached to clothing.

Based on the information received the operation didn't notify the parent that the child had been left outside unattended. The child's grandparent was notified by someone that worked in a building next door that they saw the child unattended on the playground. The grandparent notified the parent of the incident and the parent notified the director

Based on the information received during the investigation the caregiver in charge didn't make sure that all children were accounted for before coming in after outdoor play and after entering the building/classroom.
