During review all staff checked had CPR training that did not meet the hands on requirement.

The young infant room was observed to be out of ratio.

During the inspection two children were observed not washing their hands after one used the toilet and the other had their diaper/pull-up changed.

Multiple exposed nails were observed on the fences in the playground.

During inspection, 1 infant was observed asleep in a swing. This was corrected when the teacher removed the infant.

During inspection, 1 infant was observed asleep in a swing and 1 infant was observed asleep in a rocker.

During inspection, the after school class was observed watching a television show on Netflix that was not educational.

During inspection, 1 infant was observed sleeping in a bobby with a bottle in mouth.

During inspection, the after school room was observed and did not have a posted activity plan.

The operation did not use good judgment when a caregiver called a parent to come physically discipline their own child while in care at the operation.

The operation violated minimum standards and their operational handbook when not allowing a parent into the operation to pick up their child.

Acceptable behavior and self control were not taught when the caregiver grabbed a child by their arms and leaned over the child while the child was laying on the floor to reprimand that child's behavior.

Based on a DFPS investigation this standard was found to be out of compliance when the caregiver grabbed the child by their arm hard enough to cause bruising.

The caregiver did not show good judgment when they reacted to the inappropriate behavior of one of the children in their care by grabbing the child by the arm hard enough to cause bruising.
