It was determined through video surveillance that a child did not wash their hands after diaper changing.

It was determined through video surveillance that a shelf in the 2 year old classroom was not properly secured to the floor causing it to fall on a child in care.

The infant room had 11 infants with 2 caregivers where the specified age group was 0-6 months. This was corrected at the inspection when 1 infant was moved to another room bringing the operation into compliance.

It was determined through the course of the investigation that a child was left unattended in an operation vehicle for approximately 4 minutes.

It was determined throughout the course of the DFPS Investigation that neglectful supervision did occur with the child(ren) in care.

It was determined through the course of the investigation that the operation failed to obtain the parent's signature within 48 hours from the date of the incident. The incident occurred on 10/11/2024. Records indicate the parent signed the incident report on 10/22/2024.

It was determined through the course of the investigation that a caregiver did not conduct a physical walk through and visual check of the vehicle before disabling the alarm from the rear of the vehicle.

It was determined through the course of the investigation that a child was left on the playground unsupervised.

A caregiver wrapped a 2-year-old child in a blanket in order to lay the child down for naptime. The caregiver then used her hand to push the child's head down when the child lifted up and used her body to push the child's body down causing the child to fall forward and receive an injury.

Upon review of files an employee was missing an affidavit for employment and three employees were missing proof of education qualifications.

It was determined while reviewing video footage that a caregiver hit a child in the back of the head with their hand in an effort to discipline the child.

It was determined while reviewing video footage that a caregiver can be seen exiting the classroom, leaving another caregiver alone to supervise 21 three and four year old children. This occurred twice on the same day. The first time for 21 seconds and the second time for 26 seconds.

It was determined while reviewing video footage that caregiver grabbed and pulled a child by the arm in an effort to discipline the child.

A group of 33 children were observed to be playing on the playground. The youngest child was 13 months of age and the oldest child was 32 months of age. This was corrected at the inspection when the oldest group of children left the playground.

It was determined throughout the course of the DFPS investigation and after review of video footage that a caregiver lacked good judgment and self-control with a child in care when she used her arm to shove a child to the floor and then failed to comfort the crying child afterwards. The caregiver also used her hand to push a child away from her after talking to the child.
